What should I bring to my appointment?

Please bring the following items with you to your appointment.

• completed forms packet
• your insurance card
• a valid photo id
• all medications you are currently taking
• payment for co-pays and non-covered services

Can I get a same-day appointment if I am sick?

Our goal is to provide the health care you need in a timely, sensitive and caring manner. Please call our office promptly so we can help you determine if you need to be seen.
