What should I bring to my appointment?
Please bring the following items with you to your appointment.
- completed forms packet
- your insurance card
- a valid photo id
- all medications you are currently taking
- payment for co-pays and non-covered services
What is your average wait time for a well-visit appointment or annual physical?
We suggest scheduling healthy checkups 1 to 2 months in advance.
Can I get a same-day appointment if I am sick?
Our goal is to provide the health care you need in a timely, sensitive and caring manner. Please call our office promptly so we can help you determine if you need to be seen.
Does your practice use nurse practitioners and physician assistants?
We have advanced practice practitioners on staff, and they are available to help you under the doctors' supervision.
